Can you know the Lord without evidence? Can belief be enough to know something?
 
[for me anyway] we know the Lord, not by feelings, neither by intellect, nor by what others say; but by His Word: such is common knowledge to them that believe.

The evidence needed is the recognition of the Lord's voice. This is not easy, but it is easy.

"Come unto me, all ye that labour and are heavy laden, and I will give you rest.
Take my yoke upon you, and learn of me; for I am meek and lowly in heart: and ye shall find rest unto your souls.
For my yoke is easy, and my burden is light." Matt 11:28-30

When you know that you are hearing His voice, then you have the evidence, but it is your evidence. You may share your evidence with others but not everyone will hear a testimony for not everyone can hear:

"He that hath ears to hear, let him hear." Matt 11:15

What other evidence is acceptable?

The natural physical man has five senses that gives him evidence of the world in which he lives. The spiritual man (born again) also known as the hidden man of the heart (spirit) has the same senses, but they contact the spiritual realm. To walk by faith means we do not walk by what our physical senses are telling us, but what our spiritual senses are telling us. Our "evidence" does not come by way of the flesh, but by way of our spiritual senses. The evidence that comes by faith can not be seen with our physical eyes.

Recognizing other voices in my head will land me back in hospital, how do I get around this problem?

The Apostle Paul while teaching certain believers in the book of Hebrews said....

Heb 5:12 For when for the time ye ought to be teachers, ye have need that one teach you again which be the first principles of the oracles of God; and are become such as have need of milk, and not of strong meat.
Heb 5:13 For every one that useth milk is unskilful in the word of righteousness: for he is a babe.
Heb 5:14 But strong meat belongeth to them that are of full age, even those who by reason of use have their senses exercised to discern both good and evil.

These people were not dead, they were simply "baby" Christians. The only thing baby Christians know is what their "flesh" tells them, and what their physical mind can come up with. The in-depth revelations of God's Word will only be given to those who have exercised their spiritual senses to discern between good and evil.

All infants when they are first born can not control their limbs and they are constantly flailing their arms, and legs uncontrollably. They can not see clearly because they need time to develop their ability to focus their eyes. There are many believers who have been saved for decades, but have never learned to exercise their spiritual senses, probably because they have never been taught have them!!!

I do not mean to step on toes or to unnecessarily cause mental or emotional difficulties.

I do recognize the Lord's voice when I am not quenching the Holy Spirit. I cannot sort things out for you. God, of course, can. Can a psychiatric hospital? If God is using it in that way.

God does NOT speak to man's head (brain), he speaks to man's "spirit" (AKA the heart) God is a Spirit, so Spirit speaks to spirit, not flesh.

Pro 20:27 The spirit of man is the candle of the LORD, searching all the inward parts of the belly.

Psa 18:28 For thou wilt light my candle: the LORD my God will enlighten my darkness.
