What does it mean to confess?

To admit you did it. Both to yourself and to God. God already knows what you did, but you need to acknowledge that you did it.
Sometimes confession involves confessing to other people what you did.

Psa 51:1; Be gracious to me, O God, according to Your lovingkindness; According to the greatness of Your compassion blot out my transgressions.
Psa 51:2; Wash me thoroughly from my iniquity And cleanse me from my sin.
Psa 51:3; For I know my transgressions, And my sin is ever before me.
Psa 51:4; Against You, You only, I have sinned And done what is evil in Your sight, So that You are justified when You speak And blameless when You judge.
Psa 51:5; Behold, I was brought forth in iniquity, And in sin my mother conceived me.
Psa 51:6; Behold, You desire truth in the innermost being, And in the hidden part You will make me know wisdom.
Psa 51:7; Purify me with hyssop, and I shall be clean; Wash me, and I shall be whiter than snow.
Psa 51:8; Make me to hear joy and gladness, Let the bones which You have broken rejoice.
Psa 51:9; Hide Your face from my sins And blot out all my iniquities.
Psa 51:10; Create in me a clean heart, O God, And renew a steadfast spirit within me.
Psa 51:11; Do not cast me away from Your presence And do not take Your Holy Spirit from me.
Psa 51:12; Restore to me the joy of Your salvation And sustain me with a willing spirit.
Psa 51:13; Then I will teach transgressors Your ways, And sinners will be converted to You.
Psa 51:14; Deliver me from bloodguiltiness, O God, the God of my salvation; Then my tongue will joyfully sing of Your righteousness.
Psa 51:15; O Lord, open my lips, That my mouth may declare Your praise.
Psa 51:16; For You do not delight in sacrifice, otherwise I would give it; You are not pleased with burnt offering.
Psa 51:17; The sacrifices of God are a broken spirit; A broken and a contrite heart, O God, You will not despise.
Psa 51:18; By Your favor do good to Zion; Build the walls of Jerusalem.
Psa 51:19; Then You will delight in righteous sacrifices, In burnt offering and whole burnt offering; Then young bulls will be offered on Your altar.

Jas 5:16; Therefore, confess your sins to one another, and pray for one another so that you may be healed. The effective prayer of a righteous man can accomplish much.
1Jn 1:9; If we confess our sins, He is faithful and righteous to forgive us our sins and to cleanse us from all unrighteousness.

Lev 5:5; 'So it shall be when he becomes guilty in one of these, that he shall confess that in which he has sinned.
Lev 26:40; 'If they confess their iniquity and the iniquity of their forefathers, in their unfaithfulness which they committed against Me, and also in their acting with hostility against Me—
Numb 5:7; then he shall confess his sins which he has committed, and he shall make restitution in full for his wrong and add to it one-fifth of it, and give it to him whom he has wronged.
1Kin 8:35; "When the heavens are shut up and there is no rain, because they have sinned against You, and they pray toward this place and confess Your name and turn from their sin when You afflict them,
Psa 32:5; I acknowledged my sin to You, And my iniquity I did not hide; I said, "I will confess my transgressions to the LORD"; And You forgave the guilt of my sin. Selah.
Psa 38:18; For I confess my iniquity; I am full of anxiety because of my sin.

Usually when the Bible speaks of confession, it involves the sin you did. But sometimes it can simply be a proclamation of faith.

Rom 10:9; that if you confess with your mouth Jesus as Lord, and believe in your heart that God raised Him from the dead, you will be saved;

In this case it involves 3 things. Saying it with your mouth, believing it with your heart, and knowing that Jesus was raised from the dead.
For some people, it's one out of three, or two out of three.

A while back, someone close to me was trying to lose weight. I told them they snack too much between meals, and that they actually eat 5 or 6 times a day.
They didn't believe me, so I had them write down everything they ate during the day, for a week. They were quite shocked. They were embarrassed to read
it to me. I think speaking out our sins does at least three things ( if not more ) first, it is a confession ( I'm sorry God for what I did ) but second, it reminds us
how much we sin, and how big a price he had to pay for us. Finally, it can release healing to us.

Psa 32:3 ; When I kept silent about my sin, my body wasted away Through my groaning all day long.

I personally think this is one of the most underrated verses in the Bible. Oh, a lot of people know it and even quote it. But, they focus on the "forgive us" and "cleanse us" portions.
It's great that God forgives us. Don't get me wrong, without that we are lost. But a lot of people leave out the part about "if we confess our sins".
What if I don't... what if I never confess my sins? Is there are a verse in the Bible that says He will forgive us even if we don't confess?
There are certainly a few that say He won't forgive us if we don't repent.
 
That if thou shalt confess with thy mouth the Lord Jesus, and shalt believe in thine heart that God hath raised Him from the dead, thou shalt be saved.
Romans 10:9

Yea, let none that wait on Thee be ashamed: let them be ashamed which transgress without cause.
Psalm 25:3
